Medical Waiver

When purchasing any medical device, including hearing aids, you acknowledge: You are 18 years of age or older. You have been advised that the FDA recommends a medical evaluation by a licensed physician before purchasing a hearing aid, but you have chosen to waive this recommendation. You have read and agreed to the full FDA waiver provided during the checkout process. This acknowledgment is required to complete your purchase.

All content provided by MCA, whether by employees, contractors, or through our website, is for informational purposes only and does not constitute professional medical advice. Limitation of Liability

Limitations

Service satisfaction concerns must be reported within 48 hours of the Home Visit by contacting contact@medicalcentersofamerica.com. MCA reserves the right to investigate any claims thoroughly. By using our Services, you accept these Terms and acknowledge the Bill of Sale associated with your purchase.

Home Visit Checklist includes:

Personalized fitting and usage instructions. Ear examination for wax or obstruction. Instruction on device accessories. App and software guidance. Demonstration of battery replacement and device charging. Intellectual Property All content on the MCA website is owned by MCA and/or its licensors and is protected by intellectual property laws. Permitted Use

Our Services are intended for individuals 18 years or older. By using our website, you represent and warrant that you meet this age requirement. Our Services are for personal, non-commercial use only. Any unauthorized or illegal use of the Services is prohibited. We reserve the right to refuse service, terminate accounts, or cancel orders at our discretion.

Audiogram Submission You agree to:

Submit an audiogram within 10 business days of purchase, or Undergo a hearing examination conducted by our specialist during your initial fitting. No hearing aid will be dispensed without a valid audiogram. Home Visit Policy Each hearing aid purchase includes one complimentary home visit by a licensed healthcare specialist. You may waive this visit by confirming via email or phone. Additional home visits are available for a fee set by MCA.

Safety Policy

To ensure a safe environment: Secure valuables and fragile items prior to appointments. Notify us of any pets and secure them as needed. Do not request lifting of heavy objects. Harassment or inappropriate behavior toward our staff will result in immediate service cancellation. We reserve the right to cancel a visit if safety risks are identified.

Cancellation Policy

Free Cancellation: Cancel a scheduled home visit 48 hours before the appointment. Late Cancellation: Canceling within 48 hours forfeits your free home visit. Rescheduling: Reschedule a home visit free of charge if requested more than 48 hours in advance. Additional home visits can be booked for a fee as agreed upon in writing.
